Make a Difference
The Life Skills Specialist (LSS) monitors and provides care coordination and life skills training and development in collaboration with the care team to youth and/or adults with mental health and/or substance use disorders, including those with serious and persistent mental illness and chronic addiction.
Exceptional Skills And Qualifications- Collaborates as part of a multidisciplinary care team; assists with gathering data for comprehensive psychosocial assessments; able to deescalate clients displaying aggressive behaviors; documents services provided in Electronic Medical Record (EMR) based on policies and procedures; when necessary, will transport clients in personal vehicles.
- Complies with all applicable Behavioral Health Care Service policies and procedures; actively involved in professional development and education, including mandatory training; completes time cards, PTO requests, mileage reports, and other documentation per Community Behavioral Health policies and procedures.
- Individual may not perform all of these duties, or may perform additional, related duties not listed here.
- High School Diploma or GED (required)
- Current Indiana Driver's License (Required)
- Two (2) years of social service experience in a volunteer or paid role (required)
