Position Duties- Care Coordination
- Continuum of Care
- Discharge Planning
- Patient Focus and Care Plans
- Resource Utilization in an acute rehab setting
Required Qualifications
- TN/compact RN license
- BLS certification
- 2 years recent acute hospital case management experience
13 weeks
Shift / Hours per Week8:00 AM-5:00 PM with every other weekend
