Schedule: Mon-Fri 8a-4:30p
Salary: $50,000-$60,000 based on licensure
Patient Population: Adolescents ages 10-18
Licensure: RN or LPN license required
Embrace U is an evidence-based mental health provider offering group therapy, including Intensive Outpatient Programs (IOP) and Partial Hospitalization Programs (PHP), designed for adolescents ages 10-18.
ResponsibilitiesJob purpose
- The adolescent care coordinator provides quality administrative and clerical services for program staff and assists Embrace U clients with care needs. They also serve as back up coverage to other Embrace U staff. The adolescent care coordinator’s responsibilities include assisting with the inquiry and intake process, providing clerical support to team members, assisting with discharge planning, working with clients during PHP time, holding psycho educational client groups, and assisting with client care.

- The Adolescent Care Coordinator will be exposed to virus, disease, infection from clients.
- This position has no direct reports.
- RN or LPN license required.
- Education:
bachelor’s degree preferred. - Experience:
experience in providing care to adolescents preferred. - Experience performing administrative or clerical duties, working with computers and electronic medical records.
- Knowledge of community resources for adolescents.
While performing the duties of this job, the employee is regularly required to talk and hear. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception and ability to adjust focus. This position required intermittent physical activity, including standing, walking, bending, kneeling, stooping and crouching as well as lifting, driving and supporting clients.
