Become the Case Manager you always wanted to be

  • Work withinterdisciplinaryteam,guidingtreatmentplansbasedonpatientneedsand preferences.
  • Completecasemanagementaddendumsandallrequired documentation.
  • Maintainknowledgeofregulations/standards,company policies/procedures,and department operations.
  • Review/analyzecasemanagementreports,including

    Key Care Indicators ,andplanappropriate actions.
  • Attend Acute Care Transfer (ACT) meetings to identify trends and collaboratively reduce ACTs.
  • Meetwithpatient/familyper

    Patient Arrival and Initial Visit Standard within
    24hrs.of admission.
  • Performassessmentofgoalsandcompletecasemanagementaddendumwithin
    48hoursof admission.
  • Educate patient/familyonrehabilitationand

    CaseManagerrole;establish communication plan.
  • Assistpatientwithtimelyprocuring/planningofresourcestoavoiddischargedelaysor issues.
  • Monitorcompliancewithregulationsfororthoticsandprostheticsorderingand payment.
  • Make appropriate/timely referrals,includingdocumentationtopostdischarge providers/physician
  • Participateincasemanagementon-callscheduleas needed.

Qualifications

  • License
    or
    Certification
    :
    • Must be qualified to independently complete an assessment within the scope of practiceofhis/her discipline(for example,RN,SW,OT,PT,ST,and Rehabilitation Counseling).
    • Iflicensureisrequiredforone’sdisciplinewithinthestate,individualmustholdan active license.
    • Mustmeeteligibilityrequirementsfor

      CCM®orACM™certificationuponentryintothis position OR within two years of entry into the position.
    • CCM®orACM™certification required

      ORmustbeobtainedwithintwoyearsofbeing placed in the Case Manager II position.
  • Minimum
    Qualifications
    :
    • For Nursing, must possess minimum of an Associate Degree in Nursing, RN licensure with BSN preferred. A diploma is acceptable only in those states whose minimum requirementforlicensureorcertificationisadiplomaratherthanan a ssociate d egree.
    • Forallothereligiblelicensedorcertifiedhealthcareprofessionals,mustpossessa minimum of a bachelor’s degree and graduate degree is preferred.
    • 2yearsofrehabilitationexperience preferred.
