Become the Case Manager you always wanted to be
- Work withinterdisciplinaryteam,guidingtreatmentplansbasedonpatientneedsand preferences.
- Completecasemanagementaddendumsandallrequired documentation.
- Maintainknowledgeofregulations/standards,company policies/procedures,and department operations.
- Review/analyzecasemanagementreports,including
Key Care Indicators ,andplanappropriate actions. - Attend Acute Care Transfer (ACT) meetings to identify trends and collaboratively reduce ACTs.
- Meetwithpatient/familyper
Patient Arrival and Initial Visit Standard within
24hrs.of admission. - Performassessmentofgoalsandcompletecasemanagementaddendumwithin
48hoursof admission. - Educate patient/familyonrehabilitationand
CaseManagerrole;establish communication plan. - Assistpatientwithtimelyprocuring/planningofresourcestoavoiddischargedelaysor issues.
- Monitorcompliancewithregulationsfororthoticsandprostheticsorderingand payment.
- Make appropriate/timely referrals,includingdocumentationtopostdischarge providers/physician
- Participateincasemanagementon-callscheduleas needed.
Qualifications

Certification
:- Must be qualified to independently complete an assessment within the scope of practiceofhis/her discipline(for example,RN,SW,OT,PT,ST,and Rehabilitation Counseling).
- Iflicensureisrequiredforone’sdisciplinewithinthestate,individualmustholdan active license.
- Mustmeeteligibilityrequirementsfor
CCM®orACM™certificationuponentryintothis position OR within two years of entry into the position. - CCM®orACM™certification required
ORmustbeobtainedwithintwoyearsofbeing placed in the Case Manager II position.
- Minimum
Qualifications
:- For Nursing, must possess minimum of an Associate Degree in Nursing, RN licensure with BSN preferred. A diploma is acceptable only in those states whose minimum requirementforlicensureorcertificationisadiplomaratherthanan a ssociate d egree.
- Forallothereligiblelicensedorcertifiedhealthcareprofessionals,mustpossessa minimum of a bachelor’s degree and graduate degree is preferred.
- 2yearsofrehabilitationexperience preferred.
