Overview
Community Health Partner (CMA Preferred) —
Location:
Knoxville, TN
Matter Health is seeking a Community Health Partner to join our growing care team in Knoxville, TN. This is a patient-facing, community-based role focused on building trusted relationships, engaging individuals directly in care, and supporting access to high-quality healthcare services.
As a Community Health Partner, you will work hands-on with patients in the community, serving as a key connection between individuals and Matter Health’s clinical care model. This role is ideal for someone passionate about direct patient interaction, health education, and making a measurable impact on community health outcomes.
Responsibilities- Engage directly with patients in the community to educate, enroll, and support them in Matter Health’s care programs.
- Build trusted relationships through consistent, face-to-face patient interaction.
- Conduct in-person screenings to identify social determinants of health (SDOH) and document findings accurately.
- Coordinate community resources and follow up with patients to support care continuity.
- Support onsite and virtual care delivery by assisting with technology setup for patient-provider visits.
- Collaborate closely with providers, care navigators, social workers, and other members of the care team.
- Participate in and help organize community outreach events, workshops, and engagement initiatives.
- Maintain positive relationships with property stakeholders and community partners.
- Represent Matter Health professionally while promoting a welcoming, inclusive environment for all patients.
- Perform additional duties as assigned to support community and patient care needs.
- Certified Medical Assistant (CMA) certification strongly preferred.
- Bachelor’s degree or equivalent relevant healthcare or community-based experience.
- Experience working directly with patients in a healthcare, community health, or outreach setting.
- Prior experience enrolling patients into healthcare programs or care delivery platforms preferred.
- Knowledge of medical terminology and comfort working one-on-one with patients.
- Experience using digital scheduling tools and basic computer applications.
- Strong communication, interpersonal, and relationship-building skills.
- Ability to work independently in the community while collaborating with a care team.
- Passion for patient advocacy, community engagement, and improving health outcomes.
- Ability to stand and walk for extended periods while visiting multiple community locations.
- Ability to navigate uneven surfaces, stairs, and outdoor environments.
- Frequent bending, stooping, kneeling, and lifting materials up to approximately 25 lbs.
- Ability to work in varying weather conditions as this role includes outdoor activities.
- Valid driver’s license and ability to travel locally between sites.
- Occasional travel to other locations for meetings or training may be required.
