Job Description

Performs direct and indirect patient care activities under supervision of licensed personnel.

• Establishes and maintains positive interpersonal relationships and promotes effective teamwork with patients, families, coworkers and the community to meet customer service goals.
• Performs direct patient care activities while maintaining maximum comfort and safety of patients.
• Assists with admission, transfer and discharge of patients.
• Obtains and maintains accurate patient data and documents appropriately in medical records including vital signs, intake and output, height and weight, etc.
• Recognizes and communicates changes in patient's vital signs, physical and/or mental condition to the Registered Nurse.
• Provides personal care assistance with activities of daily living to patients including bathing, dressing, eating, drinking, oral care, elimination and transportation.
• Assists Registered Nurse in skin care including repositioning, cleaning of wounds, and pressure ulcer prevention.
• Demonstrates ability to assist in patient transfers effectively and safely.
• Responds appropriately to emergency situations or unplanned events.
• Assists with falls prevention activities on the nursing area, responding quickly as appropriate to enhance patient safety.

High School diploma or GED required;
Patient Care experience preferred.
