American Traveler is hiring a travel LPN for a Medically Ready for Discharge (RDU) unit within an acute care Med/Surg setting in Oregon, requiring 2+ years of experience and a current Oregon LPN license.

Job Details

• Acute care Med/Surg (Medical Surgical) setting in a hospital environment,

• Assigned to a Medically Ready for Discharge Unit (RDU), supporting patients experiencing discharge delays due to various barriers,

• Works under the supervision of an RN and collaborates with a multidisciplinary team including case managers, social workers, CNAs, and UAPs,

• Charting performed in Epic (experience required),

• 12-hour night shifts with every other weekend (EOW) required,

• 13-week contract,

Job Requirements

• Current Oregon LPN license required if applicable,

• Minimum 2 years of LPN experience required,

• Acute care experience required,

• Current BLS certification required,

• Proficiency in basic nursing skills, patient assessment, and documentation,

• Knowledge of discharge planning processes and basic patient education techniques,

• Ability to frequently stoop, bend, reach, stand, walk, and sit; must be able to lift at least 50 lbs and push/pull hospital equipment,

• Ability to function effectively during periods of stress, trauma, and/or disease,

Additional Information

• Perform focused patient assessments and implement prioritized care plan interventions as directed by the RN's plan of care,

• Assign and supervise focused care interventions to LPNs, CNAs, and UAPs in accordance with their license, certification, and patient needs,

• Accurately document focused assessments, interventions, patient responses, and progress in the electronic health record,

• Assist with patient and family education, reinforcing discharge plans, medication instructions, and follow-up care needs,

• Collaborate with the interdisciplinary team to support discharge planning and ensure smooth care transitions,

• Participate in unit-based quality improvement initiatives focused on patient outcomes and discharge efficiency,

• Every other weekend coverage is required,
