The Licensed Practical Nurse (LPN) in the Medically Ready for Discharge Unit (RDU) works under the supervision of the RN to provide direct patient care for patients who are medically ready for discharge but experiencing delays due to placement or other barriers. The LPN collaborates with the multidisciplinary team to ensure safe, timely transitions and continuity of care in accordance with Oregon scope of practice and hospital policies.
Rolesand Responsibilities
- Perform focused patient assessments within the RN-directed plan of care
- Develop and implement focused care plans as identified by the RN
- Evaluate patient responses to interventions
- Communicate patient status and concerns promptly to the RN
Task Assignment & Supervision
- Assign focused interventions to LPNs, CNAs, or UAPs within scope
- Supervise delegated tasks and ensure completion according to patient needs
- Follow RN direction for situations outside defined care parameters
Documentation
- Accurately document focused assessments, interventions, and patient responses in EPIC
- Complete all required documentation per hospital policy and regulatory standards
Patient Education
- Reinforce discharge plans, medication instructions, and follow-up care needs
- Support patient and family understanding of care transitions
- Adhere to hospital policies and protocols
- Report concerns to charge nurse or leadership
- Participate in unit-based quality improvement initiatives
- Promote safe and cost-effective use of supplies and equipment
- Current Oregon LPN License (Required)
- Minimum 2 Years of Experience
- Acute Care Experience Required
