- Guaranteed Hours: 36 hours per week
- Shift: 12-hour nights (every other weekend required) – night shift LPN
- Charting: EPIC electronic health record (EHR) – EPIC experience preferred
- Estimated Weekly Pay: $1,530 – $1,610
- Current Oregon State LPN license (or eligibility to obtain) – LPN license in Oregon required
- Current BLS (Basic Life Support) certification
- At least 1 year of acute care nursing experience required (Med/Surg or similar setting)
- Proficiency in basic nursing skills, focused patient assessment, and accurate documentation
- Experience or knowledge of discharge planning processes, patient education, and care transitions
- Familiarity with EPIC or other electronic health records (EHR) is highly desirable
- Strong communication, teamwork, and interpersonal skills for interdisciplinary collaboration
- Flexibility and adaptability in a dynamic healthcare environment focused on discharge readiness
- Physical ability to frequently stoop, bend, reach, stand, walk, sit, and lift at least 50 pounds
- Provide direct patient care as a Licensed Practical Nurse under RN supervision for patients medically ready for discharge
- Perform focused assessments and implement prioritized interventions as directed by the RN
- Develop and carry out focused plans of care addressing patient needs related to discharge readiness and safe transitions home
- Monitor and evaluate patient responses to interventions and progress toward discharge goals
- Accurately document assessments, interventions, and patient progress in EPIC (EHR documentation)
- Assign and supervise tasks to CNAs and unlicensed assistive personnel as appropriate
- Assist with patient and family education, reinforcing discharge plans, medication instructions, and follow-up care needs
- Communicate effectively with the interdisciplinary team (physicians, RNs, case managers, social workers, PT/OT) to ensure continuity of care
- Adhere to hospital policies, procedures, and regulatory standards to support safe patient discharges
- Participate in unit-based quality improvement initiatives to enhance patient outcomes and discharge efficiency
