Responsibilities & Duties

• Provide direct patient care through office visits, telehealth, and occasional same-day appointments
• Conduct thorough assessments, order/interpret labs & imaging, diagnose acute and chronic conditions
• Develop personalized treatment plans with lifestyle interventions, supplements, hormone therapies, and prescriptions
• Educate and empower patients using functional and preventive health principles
• Collaborate closely with health coaches, nutritionists, mental health providers, and clinical support staff
• Document patient encounters in Next Gen EHR
• Participate in team meetings, quality initiatives, and training sessions
• Support a positive, team-oriented clinical culture

Required Qualifications

• Active Oregon license as a Nurse Practitioner (FNP, AGNP, AGNP-PC), PA-C, MD, or DO (or eligibility to obtain)
• Board certification in Family Medicine, Internal Medicine, or related specialty
• Minimum 2 years’ clinical experience preferred
• Strong interest in preventive, functional, and lifestyle medicine
• Excellent communication and patient engagement skills
• Comfortable in a fast-paced, collaborative care setting
• Proficiency with EHRs (Next Gen preferred)
