Nurse Practitioner – Skilled Nursing Facility (SNF)

Full-Time | W2 | Weekdays | On-Site

$120,000–$150,000 annually (DOE) + Full Benefits

We are hiring an experienced Nurse Practitioner (NP) to provide high-quality medical care in Skilled Nursing Facilities (SNFs). This is a stable, full-time W2 position offering competitive compensation, strong clinical autonomy, and a predictable weekday schedule.

• Competitive W2 salary ($120K–$150K DOE)

• Perform comprehensive patient assessments in SNF settings
• Diagnose and manage acute and chronic medical conditions
• Conduct routine patient rounds
• Collaborate with physicians, nursing staff, and care teams
• Order and interpret labs, diagnostics, and treatments
• Document all patient care accurately in the EMR

• Experience in Skilled Nursing Facilities, post-acute, or long-term care required
• Strong clinical judgment and decision-making skills
• Ability to work independently while collaborating with a care team
• Comfortable managing a patient panel in an on-site setting
