Registered Nurse

Overview

Registered Nurse (PD RN) at DaVita Kidney Care. This role focuses on peritoneal dialysis education and patient care in a clinical setting, with the possibility to independently manage patient cases and dialyze patients at home.

• Provide comprehensive patient care in an independent setting and manage complete care plans for assigned patients.
• Assess, diagnose, and implement care plans while tracking and organizing patient information as a case manager.
• Educate patients and care partners to perform their own dialysis and take an active role in their health.
• Lead, coach, and guide with nursing skills; work independently while building relationships with patients, support systems, and care partners.
• Home dialysis program support, including educating new in-center hemodialysis patients in clinics.
• Coordinate home visits and on-call shifts as part of the program schedule.

• Typically, an 8-hour day, Monday through Friday with weekends and holidays off.
• Flexibility to manage your own cases and schedule.
• Scheduled home visits and on-call shifts vary by program and are compensated accordingly.

• Current Registered Nurse (RN) license in the state of practice.
• Minimum of 12 months of RN experience.
• Current driver s license, reliable transportation, and vehicle insurance in state of residence.
• Associates Degree in Nursing (A.D.N) required;
  Bachelor of Science in Nursing (B.S.N) or three-year diploma may substitute for the nursing degree.
• Basic computer skills and proficiency in MS Word and Outlook.

• Experience in Chronic Hemodialysis, Acute Dialysis, Home Hemodialysis (HHD) or Peritoneal Dialysis (PD) nursing.
• Training, teaching, or healthcare education background.
• Previous case management experience.
• Familiarity with telehealth technologies.
• CNN, CDN, or CPDN certifications.
