* Key Responsibilities:

*** Administer medications via oral, injection, and other approved methods.
* Conduct physical and mental health assessments and psychiatric screenings.
* Document medication administration and nursing assessments accurately.
* Perform MAR reviews, medication counts, and manage house stock and refills.
* Communicate with clients regarding medication needs and coordinate refill requests.
* Prepare clients for appointments by verifying medical history and collecting vitals.
* Maintain timely documentation of nursing interventions and client safety plans.
* Support lab and phlebotomy duties as needed.
* Serve as a resource for infection control, hygiene, and general health concerns.
* Participate in team meetings and contribute to continuous improvement efforts.
** Education and/or Experience

Qualifications:

*** Current licensure as a Licensed Practical Nurse (LPN) in the state of practice.
* Graduation from an accredited practical nursing program.
*
* Additional Qualifications:

*** Strong verbal and written communication skills.
* Proficient in EHR systems and documentation practices.
* Ability to interpret lab results, clinical notes, and procedural manuals.
* Skilled in basic math and medication dosage calculations.
* Demonstrates sound judgment, adaptability, and a collaborative mindset.
* Committed to confidentiality and secure handling of sensitive information.
* Comfortable working in a fast-paced, client-centered environment.
