** Key Responsibilities
*** Facilitate child and family team meetings and coordinate care with guardians and professionals.
* Assist clients in identifying and accessing resources for independent, healthy living.
* Collaborate with a multidisciplinary team to develop and implement individualized treatment plans.
* Provide direct services including case management and daily living support.
* Conduct assessments to inform treatment planning and monitor progress.
* Serve as a liaison between clients and service providers, ensuring continuity of care.
* Advocate for clients within community systems and manage crisis situations as needed.
* Participate in organizational initiatives such as quality assurance and safety activities.
** Experience and Education
Qualifications:
*** Bachelor’s degree in a Human Services field from an accredited institution.
* Minimum of two (2) years of relevant experience.
* Strong communication and interpersonal skills.
* Proficiency in computer systems and Google applications.
* Ability to manage multiple priorities and maintain confidentiality.
* Knowledge of trauma-informed care and sensitivity to diverse populations.
* A positive, adaptable attitude and commitment to teamwork.
