ER Care & Crisis Case Manager

Mark Twain Behavioral Health is seeking a full-time EMERGENCY ROOM ENHANCEMENT (ERE) CASE MANAGER for our HANNIBAL MISSOURI service area. Consider joining our MTBH Team! We offer a supportive culture of appreciation and a strong emphasis on the health and well-being of our team.

Role Responsibilities

• Perform on-site mental health screenings and assessments as needed
• Provide extensive case management and care coordination
• Serve as a client advocate
• Support consumers in their efforts to obtain access to healthcare
• Assist consumers in resolving barriers and accessing resources
• Coordinate service delivery with appropriate agencies
• Monitor client participation and progress by tracking compliance with follow-up visits
• Develop and maintain positive relationships with hospital staff, ambulatory personnel, law enforcement, and other collaborating agencies
• Complete program-specific documentation in timely fashion
• Arrange and participate in multi-disciplinary team meetings on behalf of assigned clients
• Make a mobile response when necessary, responding to consumers
• Participate in rotation with the Mobile Crisis Response Team
• Maintain consistent updated communication with Crisis Services Director for coordination of care, particularly with individuals with acute psychiatric crises and crisis stabilization referrals
• Guide treatment development with consumer and other stakeholders and maintain a clinical record to DMH certification standards
• Maintain a professional code of ethics at all times and behave professionally and courteously with all
• Represent MTBH in the community by attendance at local interagency teams as assigned
• Participate in Quality Improvement reviews and clinical case staff meetings
• Complete the Time Sheet in Bamboo

• Adhere to all clients' rights, including confidentiality

Adherence to the MTBH Ethical Code of Conduct and all policies is required.

Qualifications

• Bachelor's degree in human services field or a minimum of four years experience in dealing with persons in crisis, with consumers and their families receiving psychiatric, substance abuse, or developmental disabilities services
• Demonstrated knowledge of alcohol and drug addiction
• Minimum of four years experience working with persons in crisis is preferred
