Hospice Nurse Practitioner

Wage Range: $143,638 - $169,174 per year

Posted wage ranges represent the entire range from minimum to maximum. For jobs with more than one level, the posted range reflects the minimum of the lowest level and the maximum of the highest level. Job offers are determined based on a candidate's years of relevant experience, level of education and internal equity.

Evergreen Health is currently recruiting a full time (1.0 FTE) Hospice Advanced Nurse Practitioner to join our exceptional and growing hospice care team in Kirkland, WA. Join a dynamic group of Hospice providers in a Seattle-area healthcare system widely recognized as a national leader in end‑of‑life care.

• Provide medical oversight for hospice patients in collaboration with hospice physicians.
• Perform face‑to‑face visits for hospice patients.
• Participate in interdisciplinary team and hospice medical provider meetings.
• Participate in quality improvement projects and provide staff education and support regarding end‑of‑life care.
• Perform other duties as indicated.

Hospice or Palliative Care experience required.
