Wage Range: $143,638 - $169,174 per year

Evergreen Health is recruiting a full‑time (1.0 FTE) Hospice Advanced Nurse Practitioner to join our Hospice Care Team in Kirkland, WA.

• Provide medical oversight for hospice patients in collaboration with hospice physicians.
• Perform face‑to‑face visits for hospice patients.
• Participate in interdisciplinary team and hospice medical provider meetings.
• Participate in quality improvement projects and provide staff education and support regarding end‑of‑life care.
• Perform other duties as indicated.

Hospice or Palliative Care experience required.
