Clinical Liaison

In this role, you ll build meaningful relationships with healthcare providers, clinical partners, and families while being empowered to grow both personally and professionally. Key Responsibilities:

• Develop census as defined by targeted goals in the business plan.
• Build and nurture referral relationships within an assigned geographic territory through face-to-face meetings and relationship-building activities.
• Assist in coordinating the referral-to-admission process to ensure seamless care transitions.
• Represent A Place at Home at community events and activities to raise awareness of our services.
• Use territory market analysis to identify new business opportunities and areas for growth.
• Address barriers to admission, provide follow-ups, and manage variables affecting admission processes.
• Conduct in-services and presentations for referral sources, community groups, and others regarding our services and outcomes.

Qualifications:

• In-depth knowledge of healthcare operations, legal guidelines, and marketplace trends.
• Two or more years of experience in a clinical role within a healthcare environment preferred.
• Professional licensure required (PT, PTA, OT, COTA).
• Current driver’s license and an acceptable driving record.
