In this role, you ll build meaningful relationships with healthcare providers, clinical partners, and families while being empowered to grow both personally and professionally. Key Responsibilities:
- Develop census as defined by targeted goals in the business plan.
- Build and nurture referral relationships within an assigned geographic territory through face-to-face meetings and relationship-building activities.
- Assist in coordinating the referral-to-admission process to ensure seamless care transitions.
- Represent A Place at Home at community events and activities to raise awareness of our services.
- Use territory market analysis to identify new business opportunities and areas for growth.
- Address barriers to admission, provide follow-ups, and manage variables affecting admission processes.
- Conduct in-services and presentations for referral sources, community groups, and others regarding our services and outcomes.
Qualifications:
- In-depth knowledge of healthcare operations, legal guidelines, and marketplace trends.
- Two or more years of experience in a clinical role within a healthcare environment preferred.
- Professional licensure required (PT, PTA, OT, COTA).
- Current driver’s license and an acceptable driving record.
