Position: Program Manager, Quality and Regulatory Compliance (Home Care Services)

Wage Range: $99,216 - $158,725 per year

Posted wage ranges represent the entire range from minimum to maximum. For jobs with more than one level, the posted range reflects the minimum of the lowest level and the maximum of the highest level. Some positions also offer additional premiums based on shift, certifications or degrees. Job Summary

The Prog Mgr-EHCS Quality and Regulatory Compliance provides structure for quality improvement activities within Evergreen Health Home Care Services (EHCS), working in close partnership with medical and clinical leaders. Responsible for providing performance information in support of the Quality and Regulatory work within EHCS, including dashboards and other relevant reports. Plans and carries out complex cross functional projects using advanced PI skills and methods, working closely with a number of committees and teams as a Performance Improvement expert;

coordinates the collection and review of performance metrics in support of the department. Responsible for developing action plans in response to the need for identified clinical outcomes.

Primary Duties
  • Oversees the production, analysis, and presentation of relevant quality data to key individuals, committees, and leaders and provides guidance on opportunities for improvement. Examples:
    Provides oversight and, in collaboration with leadership, develops the Home Health or Hospice/Palliative Care QAPI programs.
  • Responsible for the production of various reports and quality information in collaboration with other departments for the Home Health or Hospice/Palliative Care programs.
  • In collaboration with leadership, identifies improvement priorities and targets, to set annual Quality goals, and facilitates improvement work as needed.
  • Decision-making regarding appropriateness of performance improvement methods, implications of data (including trends, variation, and “best practice”), and need for changes in process or practice with leadership approval.
  • Works in close partnership with the Manager of Quality & Education and other EHCS leaders. Performs concurrent chart audits to guide, support, and educate physicians and staff on documentation requirements.
  • Collaborates with the quality team to ensure that data for all external reporting initiatives is complete and accurate and submitted before established deadlines.
  • Collects process and outcomes data from a variety of sources (chart review, computer generated reports, clinic reports, and observation) to provide accurate and consistent information for review and assessment of the clinical services and core measures.

  • Licensing, Certification, Education or Experience

    REQUIRED for the position:

    • Bachelor’s degree in Nursing, Health Administration or other health related field or equivalent experience.
    • 5 years of experience in a Home Health or Hospice/Palliative Care or related healthcare setting
    • Previous experience working with quality management
    • Ability to use qualitative and quantitative data
    • Knowledge of Performance Improvement tools and methodologies
    • Maintains thorough understanding of the dynamics of major healthcare industry trends to support the integrated analysis of performance results against goals and targets
    • Knowledge of regulations pertinent to the home care setting
    DESIRED for the position
    • Master’s degree in Nursing, Health Administration or other health related field
    • Previous leadership experience
    • Direct Home Health or Hospice/Palliative Care patient care experience
    • Six Sigma Greenbelt or Lean Certification
