Wage Range: $26.91 - $43.05 per hour
Posted wage ranges represent the entire range from minimum to maximum. For jobs with more than one level, the posted range reflects the minimum of the lowest level and the maximum of the highest level. Some positions also offer additional premiums based on shift, certifications or degrees. Job offers are determined based on a candidate's years of relevant experience, level of education and internal equity.
Job Summary:The Care Management Coordinator performs a variety of duties supporting population health and clinical care management initiatives. Responsible for care management related administrative activities including, but not limited to, patient education and outreach, follow‑up phone calls for disease management, identifying patient care gaps, abstracting clinical data, retrieving relevant outside medical records, referral management, and utilization review. Functions as a communication link for the Care Management staff, providers, insurance companies, departments, and other agencies.
Works with clinical providers and other key stakeholders by supporting data collection processes and making recommendations to improve data collection methods, accuracy, and reporting. Participates in performance improvement activities in support of department and organization goals.
Experience:
REQUIRED for the position:
- High School graduation or equivalent
- Current license as LPN in the State of Washington; or current WA State Medical Assistant Certification and current national certification through the American Association of Medical Assistants (AAMA), American Medical Technologist (AMT), National Health career Association (NHA), American Medical Certification Association (CMAC), or National Center for Competency Testing (NCCT); or commensurate clinical experience
- 2 years of experience in health care, acute care hospital or ambulatory clinic
- Experience with electronic medical record (EMR) system
- Must maintain license as appropriate to profession, if applicable
- Current Washington State Driver’s license and proof of insurability
- Reliable transportation
- Knowledge of medical terminology
- Experience with quality metrics to include HEDIS and STARS
- Ability to interact with others in a professional and courteous manner
