Are you passionate about delivering high‑quality person‑centred care in innovative settings? East Dunbartonshire is launching a new Hospital at Home Service, and we are seeking enthusiastic and dedicated Advanced Nurse Practitioners/aspiring motivated Trainee Advanced Nurse Practitioners to join our values‑driven multidisciplinary team. Our service will provide hospital‑level care to patients living with frailty in the comfort of their own home, including care homes, and will contribute to reducing unnecessary hospital admissions and supporting early discharge.
Responsibilities- Use advanced assessment, reasoning, interpretation and decision‑making skills to reach a working diagnosis.
- Work collaboratively with the responsible medical officer to plan and deliver appropriate treatments.
- Access hospital‑level diagnostics and complete necessary hospital‑level interventions at home.
- Provide continuous assessment and decision‑making as part of the wider multidisciplinary team.
- Lead discharge planning and link with local community services for collaborative care during admission to hospital at home and safe discharge.
- Qualified Advanced Nurse Practitioners or individuals with passion for advanced practice and entry onto the SWISS register.
- Trainee applicants must have experience at practitioner level and have attained a minimum of two modules from the Advanced Practice programme.
- Demonstrate competence in: clinical assessment and diagnostic reasoning, clinical judgement and decision‑making, anatomy and pathophysiology, non‑medical prescribing (V300), leadership in care delivery and evaluation, practice‑based learning or equivalent transferable modules.

As from 1st April 2026, the Agenda for Change full‑time working week will be reducing from 37 to 36 hours per week. Part‑time hours will be reduced pro‑rata. However, there will be a corresponding increase in the hourly rate so that pay will not be affected.
