This Nursing position is part of the Community Commitment Program for Nurses (CCPN) which provides a $25,000 incentive to eligible nurses in exchange for a 24-month commitment to an eligible employer

RESPONSIBILITIES:

  • Primary contact for a client when designated as the primary care provider/clinician for the client’s team
  • Provide continuous and comprehensive support to your team in the delivery of care for your client
  • Collaborate with the interdisciplinary team on the development of Complete a client assessment and develop of client centered care plan based on best evidence/practices and on client’s unique needs.
  • Facilitate regular interdisciplinary team communication (e.g. huddle, rounds, status updates) when designated as the primary care provider/clinician
  • Support discharge and transition care planning discussions during regular integrated team meetings when designated as the primary care provider/clinician
  • Facilitate and organize family and caregiver meetings
  • Support the client through care transitions by collaborating and connecting the client with community support services in their local community.
  • Support clients, families and caregivers to develop strong self-care strategies, anticipate and address health issues, as well as optimize their health through effective health promotion strategies

Requirements:

  • Current registration with the College of Nurses of Ontario
  • Canadian Nurses Association (CNA) specialty certification an asset
  • Enthusiasm and love of community care and passion for evidence-informed practice & excellence in care
  • A current CPR certificate is required
  • A valid driver’s license and vehicle (travel required)
  • Updated immunization records, clear criminal background check, and Vulnerable sector check are required upon hire.
  • A willingness to attain required Saint Elizabeth certifications.
  • Availability to days, evenings, nights and every other weekend.
  • This is a physically demanding position and requires hand, arm and shoulder dexterity, sensory ability, lifting to a maximum of approximately fifty-five (55) pounds, walking, climbing stairs, standing, bending, reaching, and frequent movement
