Assistant Practitioner

Norfolk and Suffolk NHS Foundation Trust (NSFT) is seeking a motivated and compassionate Assistant Practitioner to join the Community Mental Health Team on a maternity cover basis.

Working as part of a multidisciplinary community mental health service, the Assistant Practitioner will support the delivery of high-quality, recovery-focused care to adults with mental health needs. The post holder will work under the direction and supervision of registered practitioners, contributing to the assessment, planning, implementation, and evaluation of care.

The role involves regular direct contact with service users in the community, supporting agreed care interventions, monitoring mental and physical wellbeing, promoting independence, and contributing to risk management and care coordination. The Assistant Practitioner will also support effective communication with carers, families, and partner agencies, ensuring continuity and safety of care.

This is an excellent opportunity for an Assistant Practitioner to gain experience within a community mental health setting, develop clinical skills, and contribute to a supportive and collaborative team during a period of maternity leave cover.

Work as a member of the Community Mental Health Team, supporting the delivery of safe, effective, and person-centred care to service users in the community under the supervision of registered practitioners.

Support the assessment, planning, implementation, and review of care plans by carrying out delegated tasks and interventions in line with individual needs and recovery goals.

Undertake direct work with service users, including monitoring mental and physical wellbeing, observing and reporting changes in presentation, and promoting engagement with services.

Support risk assessment and management by recognising and escalating concerns relating to mental state, safeguarding, or risk to self or others in accordance with Trust policies.

Assist service users with practical and therapeutic interventions, including health promotion, psychoeducation, relapse prevention support, and encouragement of daily living skills.

Participate in community visits, clinics, and reviews alongside members of the multidisciplinary team.

Work collaboratively with carers, families, and partner agencies, contributing to effective communication and continuity of care.

Follow NSFT policies, procedures, and professional standards, ensuring confidentiality, dignity, and respect at all times.

Participate in supervision, training, and ongoing development to maintain competencies relevant to the Assistant Practitioner role.
