• Provide high-quality care coordination for older adults, developing and implementing recovery plans tailored to individual needs.
• Collaborate closely with multidisciplinary teams to ensure comprehensive support and effective communication between services.
• Conduct assessments to determine service users’ needs and recovery goals.
• Maintain detailed records and implementation reports to track progress and adjust care plans as necessary.
• Engage with families and carers to provide necessary education and support, fostering a holistic environment for recovery.
