Program Specialist:
They are responsible for creating individualized programming for everyone on their caseload. Programming must be directly related to and relevant to the person’s Individual Support Plan (ISP). A successful Program Specialist must possess compassion and the ability to maintain a healthy rapport with all individuals on their caseload. They must have the ability to develop programming that utilizes the strengths and skills of those individuals we serve and has milestones to allow the individual to experience levels of success/accomplishment, as well as following the company’s standards and mission.
They must be able to work as a team member and accept input from other team members. They must be organized and dependable. They ensure services are delivered successfully and add the highest possible value to the organization. They work collaboratively with agencies, families, and individuals in the coordination and implementation of each person’s Individual Support Plan (ISP).
- Foster and maintain supportive, professional relationships with individuals served, up to a caseload of 20.
- Evaluate incoming referrals to assess program fit and home compatibility
- Track all referrals and discharges
- Collaboration with the Admissions team
- Lead meet-and-greet coordination
- Ensure regulatory documentation is complete and accurate
- Conduct annual assessments and develop personalized programming tied directly to ISP objectives.
- Provide services in full compliance with state/federal guidelines and applicable service definitions
- Ensures the ISP is relevant, comprehensive, and up to date
- Create Service Notes that directly relate to each person’s ISP (keep originals)
- Maintain accurate and timely documentation:
- o Service notes
- o Quarterly progress reports
- o Annual ISP reviews
- o Updates to individual binders
- Advocate for high-quality services while professionally addressing individual concerns.
- Attend and participate in monitoring's, audits, ISP meetings, and quarterly reviews.
- Coordinate & is present for monitoring's, ISP, Quarterly reviews, and audits
- Coordinate closely with families, guardians, and Support Coordinators to maintain continuity of care.
- Review daily documentation (e.g., shift reports, service notes) to ensure program effectiveness
- Compile a comprehensive quarterly progress note to identify each person’s progress
- Lead annual ISP training for staff and ensure service notes reflect updated support plans
- Maintain a binder with updated Progress Specialist information for each person on their caseload
