PositionCare Coordinator (Full-Time, Hybrid). This is a full-time, hybrid position that requires an average of 1-2 days per week to be spent working from our offices in King of Prussia and/or Phoenixville, PA. The remainder of the work can be performed remotely.
Key ResponsibilitiesThe Care Coordinator will serve as the primary point of contact for clients, families, and care providers, ensuring seamless coordination of services and support.
Key responsibilities include:
- Client Management: Conduct initial assessments and regular check-ins with clients to understand their needs, preferences, and goals. Develop and maintain personalized care plans.
- Coordination of Services: Schedule and coordinate all aspects of client care, including appointments, necessary equipment, and specialized services. Act as a liaison between clients and external healthcare providers, specialists, and community resources.
- Communication & Support: Maintain open, clear, and professional communication with clients, their families, and the internal care team. Provide emotional support and guidance to clients and families navigating care challenges.
- Documentation & Compliance: Maintain accurate, up-to-date client records and documentation in accordance with all regulatory and company standards. Ensure compliance with care protocols and organizational policies.
- Administrative Tasks: Manage care schedules, process necessary paperwork, and assist with billing inquiries related to coordinated services.
- Office Presence: Attend required team meetings, trainings, and client-related consultations at the King of Prussia and/or Phoenixville, PA office locations 1-2 days per week on average.
- Education: High school diploma or equivalent required;
Associate's or Bachelor's degree in healthcare administration, social work, or a related field preferred.
Candidates on a gap year between undergraduate and medically related graduate education are strongly encouraged to apply. - Experience: Minimum of 1-2 years of experience in care coordination, case management, patient advocacy, or a similar role in a healthcare or social service setting.
- Skills: Excellent organizational, time management, and problem-solving abilities. Exceptional interpersonal and communication (written and verbal) skills.
- Technical Proficiency: Proficient in using electronic health record (EHR) systems and Google Suite (Docs, Sheets, Gmail).
- Location: Must reside within commutable distance of the King of Prussia and/or Phoenixville, PA offices to fulfill the hybrid requirement.
This is a full-time position offering a competitive hourly wage based on experience:
Salary Range
$16.00 - $21.00 per hour
