.
Manages the clinical and production pharmacy services provided to assigned patients. Checks filled orders, supplies, pharmacy technicians and other pharmacist's work. Sterile compounding as needed or assigned.
Hours:
Monday through Friday, 8:00 AM–6:00 PM, with required shift rotation, weekends, holidays, and on‑call.
- Provide clinical decisions regarding patient‑specific pharmaceutical care including assessment of general pharmaceutical needs, review of medication profiles, evaluation for duplicate therapies, drug interactions (drug–drug, drug–nutrient, drug–disease), and development of patient database and documentation in the medical record.
- Ensure safety of patients and pharmacy personnel.
- Provide in‑service programs to all personnel on any aspect of drug therapy and related infusion devices.
- Participate in Penn Home IT Continuous Quality Improvement program.
- Accept physician prescription orders and evaluate appropriateness for dosage, drug choice, drug interactions, route of administration, stability, compatibility, and mode of administration (e.g., infusion device).
- Monitor patient response to treatment by subjective and objective assessment, monitor laboratory tests, discuss results with nurses and physicians, and provide recommendations based on lab evaluations.
- Effectively communicate and document actions with physicians, nurses, and patients in the hard patient chart and software database, ensuring complete patient database for relevant information.
- Maintain required documentation as required by State and Federal government and JCAHO (e.g., prescription logs, medication profiles, patient charts, narcotic receipts, state pharmacy permits).
- Pharmacist license required.
- Pennsylvania State Pharmacist License required.
- Bachelor of Pharmacy (required) with a Bachelor of Science Degree in Pharmacy or Pharm
D from an ACPE‑accredited school of pharmacy (required). Minimum 1 year of experience with infusion therapies in hospital or home settings and experience in patient care, medication counseling, preparation of parenteral products, drug therapy monitoring, clinical laboratory evaluation, drug and medical supply procurement, and quality assurance. - Graduate of an accredited School of Pharmacy with 2+ years of experience with infusion therapies in hospital or home settings and equivalent experience in patient care, medication counseling, preparation of parenteral products, drug therapy monitoring, clinical laboratory evaluation, drug and medical supply procurement, and quality assurance.
