As a Nurse Practitioner with Trumen
, you will:

• Work closely with the Medical Director to develop and manage treatment plans.

• Build strong relationships with Skilled Nursing Facility (SNF) administrative and clinical teams.

• Act as a trusted liaison between SNFs and Trumen Physicians.

• Support Trumen's Value-Based Care Programs
  , helping patients transition smoothly between care settings.
