Expected pay for this position is $29.30 - $34.47/hour. Actual pay will be determined by experience, skills and internal equity. This is a Salaried position.
Work one day from home after first 90 days.
The Ambulatory Complex Care Manager promotes the achievement of optimal clinical and resource outcomes by assessing, planning, implementing, coordinating, and monitoring interventions and develops a comprehensive plan of care for the patients. Performs ongoing communication and works to promote quality of care through collaboration with the Interdisciplinary Care Team. Health care services are coordinated on behalf of the patient to promote the delivery of care at the appropriate level of care which will optimize clinical outcomes and satisfaction with services.
The Ambulatory Complex Care Manager assesses and intervenes to address the complex psychosocial and behavioral health issues and needs associated with population management and transition plans. Demonstrates the knowledge and skills necessary to provide services to the assigned patient population. Works with the Care Management Division leadership team to ensure alignment with the strategic goals to meet OSF Key Results.
Education
Bachelor’s degree in Social Work (BSW)
Licensure / CertificationsSocial Work
Shall comply with Illinois and/or Michigan state licensing and certification requirements or jurisdiction(s) in whichever state he/she practices.
- Mission Partner is required to identify clinical supervisor by day 91 of hire with a plan to complete the supervision within 3 years
- Mission partner is accountable to ensure completion of state application form once the 3 year clinical supervision is completed
- The mission partner must submit state application within one month
- All BSW mission partners must obtain licensure within 4 years from date of hire
- If a mission partner comes in with documented supervised clinical hours those will be deducted from the 4 year time requirement
- Must start licensing application process by day 91 of hire
- All MSW mission partners must obtain licensure within 1 year from date of hire
3 years of experience caring for patients
Other Skills / KnowledgeDemonstrated broad knowledge of chronic illness and medical and community resources.
Demonstrated working knowledge of a clinical EMR system and Microsoft Office applications.
Flexible work schedule—weekend, holiday, and on‑call rotation.
Requires the ability to successfully use independent judgement and make decision and requires little supervision.
Demonstrated substantial interpersonal skills to develop and maintain cooperative working relationships with personnel from other departments, with excellent time management skills.
Positive, empathetic approach and ability to communicate effectively to patients/families with a variety of health issues, age groups and socioeconomic backgrounds.
Master’s degree in Social Work (MSW)
ExperienceExperience with chronic, complex illnesses adult and/or pediatric care management
