Licensed Practical Nurse (LPN) – DaVita Skilled Nursing Dialysis (DSD)

DaVita seeks Licensed Practical Nurses to deliver dialysis care in skilled nursing facilities (SNF). In this role you will support patients with end‑stage renal failure or chronic kidney disease, working independently and collaboratively across DaVita’s new Skilled Nursing Dialysis Services line.

• 3–4 days a week, 12+ hour shifts

• Possible travel to additional SNF sites and alternating Saturdays

• No on‑call duty

• Comprehensive paid training program and ongoing professional development

• Provide full, ongoing patient care to ensure dialysis treatments remain on schedule.
• Work alongside dialysis RNs to deliver safe, comfortable, and hygienic dialysis care.
• Manage complex patient needs, including multiple co‑morbidities, and collaborate with SNF partners.
• Participate in DaVita’s award‑winning training programs, both classroom and hands‑on, to develop and maintain technical proficiency.
• Support other local SNF sites as needed to expand the service line.

• Current LPN‑LVN license in state of practice.
• Current CPR certification.
• IV certification if required by state regulations.
• High school diploma or equivalent.
• Minimum one year of nursing experience in a hospital or clinical setting (preferred).
• Basic computer skills and proficiency in MS Word, Outlook, and DaVita’s internal systems.
• Successful completion of pre‑employment color‑blind test (accommodations may be provided).
