Job overview

The CATCH Team are part of the Integrated Specialist Nursing team comprises of the CATCH Team (Community Assessment and Treatment of Children at Home), Long Term Conditions, Training, Home Respite and the Nurse led Continence Service.

Currently open Monday to Friday until September 2026, the CATCH Team provide a service from 8am-9pm 7 days a week. The post holder will work as part of a team assessing acutely unwell children in their own homes/community, taking referrals from GPs, EMAS and the acute hospital. With the aim to reduce hospital admissions and manage the child safely within the community setting.

The post holder will work as part of a team whilst working autonomously in practice in both GP hubs and community settings as first contact assessing, diagnosing & managing and prescribing for children under 6 years of age presenting with acute illness, along with providing an alternative to hospital admission, utilizing acute assessment skills and intervention on a flexible basis; educating and empowering parents in order for safe effective care to be provided within the home setting.

The post holder will be part of a telephone on call rota, along with an expectation that the post holder will gain skills to enable them to cross work with the wider Integrated Specialist Nursing Team & will take part in End of Life Care whilst being supported by wider team.
