Position: Registered Nurse Medical Speciality (Outpatient Care)
The position is at the Kernersville Health Care Center (HCC) in Kernersville, NC.

Delivers fundamental knowledge-based care to assigned clients while developing technical skills. Assesses, plans, implements, and evaluates care based on age-specific components. Assumes responsibility for the coordination of care focused on patient education, self- management, and customer satisfaction throughout the continuum of care.

Assigned Duties:

* Medical Specialty Clinic Staff Registered Nurse (RN) is responsible and accountable for individual nursing practice and determines the appropriate delegation of tasks consistent with the nurse's obligation to provide optimal outpatient care.

* Medical Specialty Care Outpatient Staff Registered Nurse (RN) is responsible for providing competent, evidence-based outpatient care to assigned patients and oversight of licensed vocational nurses/nursing assistants, as appropriate to the setting.

* Medical Specialty Clinic Outpatient Staff Registered Nurse (RN) executes position responsibilities that demonstrate leadership, experience, and creative approaches to management of complex specialty care.

* Assumes responsibility for the coordination of care focused on patient transition through the continuum of patient care, patient and family education, patient self-management after discharge, and supporting factors that impact customer satisfaction.

* The nurse will assist in directing the provision of nursing education, orientation, competencies and providing quality improvement and outcomes utilization consultation.

* This nurse provides patient care, care coordination, health promotion, health education and coaching on wellness, disease prevention, and chronic care management.

* The focus is on individual growth and development in practice with increased levels of self-direction.

* Demonstrates leadership in delivering and improving holistic, outpatient care through collaborative strategies with others.

* Collaborates in the development, implementation, evaluation, and revision of policies, procedures, and/or guidelines.

* Ability to formulate a plan of care based on nursing assessments.

* Seeks guidance on how to improve care provided to patients and families. Delegates elements of care to the appropriate individual based upon assessed needs, condition, and safety of the patient.

* The nurse brings principles and theory with limited application of critical thinking skills.

* Evaluates practice in an ongoing process, based on best evidence. Provides peers with informal constructive feedback for improvement.

* Fosters a safe and supportive environment conducive to the professional development of health care professionals.

* Contributes professional nursing perspective in discussions with the interdisciplinary team.

* Specialty care program or service level outcomes must be broad and complex and can be demonstrated at any organizational level within a facility.

* All other duties as assigned.

Pay:
Competitive salary, regular salary increases, potential for performance awards
