Life Skills Coach

Responsible for supervision and training of individuals with developmental disabilities. Works closely with the Case Manager & Program Manager to assist individuals in the development of their skills to their full potential, fostering independence in a positive and caring manner. Completes required documentation.

• Use the established methodology for achieving client goals and track progress.
• Provides care, supervision, instruction and/or job coaching for the individuals we serve; services may be provided in person, remotely or in the community.
• Participates in weekly online videos.
• Develop activities, curriculum and schedules for programming in person or remotely.
• Assists the Case Manager in implementation of the Individual Service Plans (ISP’s)/Individual Habilitation Component (IHC) and provides documentation as directed by the Program Manager.
• Assists with personal hygiene, toileting, feeding, and ambulation needs as needed.
• Accurately and legibly evaluates and documents the individuals’ progress through daily charting and log notes in the client electronic file.

• High school diploma or GED. Prefer three to six months of related experience working with developmentally disabled individuals or other special populations.
• Must possess a valid California Driver’s License and be able to pass a background check, physical, TB, and drug test.
• Must be able to push and pull up to 50 pounds.
• Must be able to use a computer for daily notes, creation of You Tube videos, and video conferencing.
