General Summary

Under the supervision of the Director of Social Services, is responsible for the initial assessment, case management, and coordination of the National Family and Kentucky Caregiver programs.

JOB TITLE: Caregiver Coordinator

SALARY RANGE: $28,000 - $38,000 DOE

General Duties And Responsibilities
  • Assist with pre-screening/intake to connect the consumer to appropriate services.
  • Facilitate program enrollment for publicly funded services.
  • Record all contacts into the data system at the time of contact and make sure all critical report data is complete.
  • Maintain a case file that includes all documentation regarding the client served in a secure location.
  • Assist in adding consumers to a waitlist, if necessary, within the state approved data system and internal tracking record.
  • Process program vouchers used for respite and supplemental services.
  • Coordinate with grandparents approved to receive supplemental services to shop for approved supplies at retail locations.
  • Attend job development training and staff meetings as requested.
  • Respond to calls from the public and provider agencies concerning programs, data, and resources.
  • Identify service needs, resources and barriers as they relate to older adults and persons with disabilities.
  • Conduct home visits with participants upon initial enrollment, recertification, and when deemed necessary.
  • Participate in and assist with coordinating support groups, health fairs, caregiver events and outreach events.
  • Make reasonable effort to understand the Older Americans Act, state policies and regulations.
  • Assist in maintaining a tracking method for funds utilized within the caregiver programs.
  • Communicaiton with financial staff as needed.
  • Maintain a computerized client tracking system and contact sheet.
  • Assist with program expansion if funding allows.
  • Perform other duties as assigned.
Minimum Qualifications
  • High school diploma or equivalent and preferred three years of direct experience working with the elderly or disabled.

Working Conditions

Most work is performed in an office, but some are performed throughout the ADD service region. Some overnight travel may be required for training purposes. This includes traveling under potentially adverse weather conditions and into rural areas to visit client homes.

Effort

Must be able to walk, see, hear, and speak. Intermittent sitting, standing, stooping, sorting and reaching. Occasional light lifting, which can be accommodated. Work requires a high level of mobility.

Machines, Tools And Equipment

Must have a valid driver’s license. Typical use of computer, scanner, printer, calculator, copier, shredder, etc. as well as an automobile. The use of Microsoft 365 products is a must.

About FIVCO Area Development District

This organization shall be known as the FIVCO Area Development District, a local agency created under the Kentucky Revised Statutes (KRS Chapter, 147A), with its purpose being to promote, develop and protect the economy, health, education, and general welfare and to implement projects in accordance with KRS 147A for the people of Boyd, Carter, Elliott, Greenup and Lawrence Counties in Northeastern Kentucky, and to further act in conjunction with adjoining Area Development Districts to implement programs of benefit to the Northeastern Kentucky area.

The FIVCO Area Development District was founded in 1968 and proudly serves its five-county region in many different capacities. The Board of Directors consists of 25 members from the five-county region. The purpose of the ADD is to assist in these areas:

  • Revolving Loan Funds
  • Tourism & Recreation
  • Aging & Disability Services
