Clinical Resource Nurse

Overview

• Assess patients for discharge needs, utilizing pre-established criteria, and plan appropriate discharge planning for the patient, involving the patient and family in the plan of care.
• Make patient rounds with the physician and nursing staff, if necessary.
• Chart audits and reviews.
• Prepare reports as outlined in the job description.
• Attend Utilization Review meetings.
• Attend daily Length of Stay meeting.
• Follow criteria as outlined by the Kentucky Medical Review Organization, which mean reviewing 100% of all Medicare charts daily for severity of illness and intensity of service.
• Comply with HIPAA regulations.
• Participate in the Hospital’s Quality Service Management Plan.
• Assist Physician in maintaining records, if necessary.
• All other duties as outlined in the job descriptions or as assigned.

• Must be a Registered nurse, with an active Kentucky License in good standing, with no restrictions.
• Must have at least 3 years nursing experience.
• Good communication skills, both written and verbal.
• Ability to work well with physicians and ancillary departments.
• Capable of functioning independently.
• Must have computer skills.
