Integrated Care Nurse Practitioner; Hybrid

Position: Integrated Care Nurse Practitioner (Hybrid)

Schedule: Monday–Friday | Flexible hours

Hybrid Role: Guaranteed 1 day per week remote

Travel: Statewide travel ~80% of the time (4 days/week). Occasional overnight travel (company paid)

Compensation: Base salary +
15% quarterly bonus

We are seeking Nurse Practitioners or Physician Assistants who enjoy variety and flexibility. This role supports patients with chronic and complex conditions, including kidney disease, through a mix of statewide in-home visits and telehealth
. Youll work as part of an integrated care team alongside a Registered Nurse and Social Worker.

• Conduct comprehensive assessments and annual wellness visits (home & virtual)
• Develop and manage individualized treatment plans
• Prescribe medications and order diagnostics
• Coordinate care with physicians, specialists, and interdisciplinary team members
• Educate patients and families on disease management and preventive care
• Complete timely, accurate clinical documentation

• Active Nurse Practitioner or Physician Assistant license and board certification
• Active DEA license
• 2+ years of direct patient care experience
• Ability to practice independently
• Willingness to travel statewide (overnight travel paid)
• Valid drivers license and reliable transportation
