• Consults and coordinates with healthcare team members to assess, plan, implement, and evaluate patient care plans.
• Prepares and administers prescribed medications orally, subcutaneously, or via IV and records them.
• Records patient medical information and vital signs.
• Monitors and adjusts specialized equipment and interprets electronic displays.
• Initiates corrective actions for adverse symptomatology.
• Provides bedside care for a variety of medical patients.
• Changes dressings, inserts catheters, and starts IVs.
• Prepares equipment and assists physicians during examinations and treatments.
• Responds to life‑saving situations according to nursing standards and protocols.
• Participates in discharge planning.
• Utilizes time‑management skills.
• Records all care information concisely, accurately, and completely in a timely manner.
• Performs other position‑related duties as assigned, depending on the setting.

• Current LPN or RN license in the State.
• Up‑to‑date first aid and CPR certification.
• Minimum of one year of LPN or RN experience in a long‑term care setting (preferred).
• Knowledge of Matrix Care preferred.
