Job specification

• Deliver comprehensive medical care to residents, including diagnosis, treatment, and preventive care.
• Collaborate with an interdisciplinary team to develop and implement personalized care plans.
• Conduct regular patient assessments and manage chronic conditions to optimize health outcomes.
• Educate residents, families, and facility staff on treatment options and healthcare management strategies.
• Perform medical procedures, including diagnostic testing and wound care, within the NP scope of practice.
• Ensure compliance with all federal, state, and facility regulations and standards of care.

Candidate requirements

• Current and unrestricted Nurse Practitioner license in Kentucky.
• Master's or Doctoral degree in Nursing.
• National board certification as an NP (Family, Adult-Gerontology, or Geriatrics preferred).
• Minimum of 1-2 years of experience in a long-term care or geriatric setting.
• BLS certification is required; ACLS is preferred.
• Strong interpersonal and communication skills with the ability to build trust and rapport with residents and staff.
• Demonstrated ability to work effectively in a team-oriented healthcare environment.
