Position: Participant Directed Services (PDS) Service Advisor/Case Manager
Location: Russell Springs

Overview

Under general direction, provides case management activities for individuals receiving PDS and blended services; serves as liaison between the participant and DAIL and Medicaid; performs related duties as required.

Training and Experience

A Bachelor’s Degree in a Health or Human Services field from an accredited college or university with a minimum of one year of experience in a Health or Human Services field or the educational or experimental equivalent in the field of aging or disabilities; or a Registered Nurse with at least two years of experience as a professional nurse in the field of aging or disabilities;

or a Master’s degree in a Health or Human Services field from an accredited college or university.

General Duties and Responsibilities

Essential:

  • Assists DAIL with outreach to and enrollment of participants into PDS.
  • Assists the participant with development and revision of the PDS plan of care / support spending plan (POC / SSP) utilizing person centered planning process and guiding principles.
  • Assists with training of participants and their contracted employees on the principles of self-determination.
  • Facilitates background checks of PDS contractors.
  • Maintains all records and documents, including case notes.
  • Assists the participant with purchasing goods / consumable medical supplies based on the participant’s approved POC / SSP utilizing a voucher system.
  • Coordinates with traditional service providers to ensure reassessments are done timely.
  • Initiates Corrective Action Plans when needed.
  • Provides technical assistance to the Participant with managing the budget, spending and maintaining contractor records.
  • Assists the participant with development of an emergency back-up plan which may include arranging for the provision of emergency services if necessary.
  • Requests authorization of additional services and funding if needed.
  • Assists the participant in hiring, training, scheduling, and terminating service providers.
  • Assists the Participant in locating service providers and negotiating rates.
  • Processes contractor enrollment forms and forwards to the Financial Management Agency and to DMS for approval.
  • Assists the participant with the transitions to PDS services and coordination of PDS, traditional waiver services, state plan services and community resources.
  • Monitors the participant’s compliance with the POC/SSP and provision of services to ensure that service provision is within the scope of the POC/SSP and prior authorization limits.
  • Forwards information to the Financial Management Agency for the processing of payroll.
  • Assures the health, safety and welfare of the participant and compliance with PDS guidelines.
  • Reports all qualifying Critical Incidents as prescribed by DAIL.
  • Recommends termination of the participant from PDS when attempts to correct fail or when there is imminent danger to the participant’s health or safety.
  • Assists the participant with transition to traditional services upon request of the participant or upon involuntary termination of PDS.
  • Conducts quarterly reviews and updates of the participant’s spending plan.
  • Monitors satisfaction of the quality of service provided.
  • Maintains confidentiality and observes HIPAA.
  • Observes established agency policies and procedures.
  • Performs other duties as assigned.
