Job Description
Alta Staff is a staffing agency currently looking for a HEDIS Health Educator to work with our Managed Care Client!
Qualified candidates must be a Licensed Clinical Social Worker (LCSW).
Pay Rate: $31.00 - $33.00 hourly
Shift:
Monday - Friday, 8-5 or 9-6 EST or CST
This position is fully remote, but candidates must reside in Kentucky.
This position will primarily be making outreach phone calls to Medicare members to support closure of HEDIS gaps in care. The Health Educator will search for alternate phone numbers for members and document all phone calls.
Job Responsibilities (but not limited to):- Act as a resource to the health educators in the resolution of issues that may arise during the implementation of any intervention protocol.
- Design, implement, and evaluate health education interventions to meet the needs of the targeted population based on priorities established by the department and current contracts.
- Act as a lead in the development of internal resources in collaboration with the Resource Specialist as identified within the annual work plan.
- Function as a liaison to internal departments, community-based organizations, social service agencies, and public health departments as assigned to ensure that resource information is current and available.
- Coordinate the health education services assessment of IPAs and medical groups as required and collaborate with QI on oversight.
- Develop effective relationships with providers, members and other external parties.
- Must be a Licensed Clinical Social Worker (LCSW).
- Must have 1+ years of experience in healthcare/clinical background.
- Previous HEDIS experience.
- Must be comfortable with a high call volume.
- Interpersonal, analytical, organizational and independent decision‑making skills.
