We invite you to join our community nursing team as a Diabetes Specialist Nurse, based in Herne Bay with caseloads across the Canterbury locality. This role is part of the adult long‑term services directorate and supports the NHS 10‑Year Plan’s focus on community‑based, preventative care.

• Provide evidence‑based, personalized diabetes care to individual patients, aiming to improve self‑management and minimise complications.
• Act as a non‑medical prescriber, working with patients and GP practices to optimise treatment plans.
• Deliver education, support and advice to patients and their families about diabetes management.
• Lead specialist education and training for other healthcare professionals in diabetes care.
• Collaborate with multidisciplinary teams (GPs, mental health professionals, social workers, voluntary services) within integrated neighbourhood teams.
• Utilise digital tools such as virtual wards to enhance patient care and reduce administrative burden.

• Strong background in contemporary diabetic evidence‑based practice.
• Non‑medical prescriber (NMP) qualification – a development programme will be available for 12 months if suitable for Band 6.
• Full UK driving licence (driving may be required for the role).
