Position: Home First Support Worker | Community Health NHS Foundation Trust

You can help people in your community and start a great new career by becoming a Support Worker for the NHS and Kent County Council Home First Team.

These are roles working across health and social care to provide support for people leaving hospital to help them returnhome safely and continue their recovery in their own surroundings.

You’ll help people to regain their confidence by helping with personal care, eating and drinking and staying as active asthey can.

You’ll have the full support of a large team of health and social care professionals at all times and your bestreward will be the smiles on the faces of the people you help to get home, where they belong.

You don’t need any formal qualifications, we’ll give you all the training you need to provide a truly valuable service topeople in your community. So, when someone in hospital says “I just want to go home”, you’ll be there to help them do just that.

We are looking for staff who:
  • Believe in providing personalised support
  • Can work independently and flexibly
  • Respect privacy and dignity and who want to expand their knowledge in both a Health and Social Care setting

No experience in the Health and Social Care sector is required, however the ability to work flexibly and a passion to makea difference is a must.

The operational hours of the service are 08:00 – 20:00 seven days a week (working on a shift basis). Contracts will be available for all staff.

This role is subject to holding a Full UK Driving Licence – The Trust & Council are committed to making reasonable adjustments so whilst this job requires the jobholder to drive your application will still be considered if you are unable to drive due to a disability.

at this stage, the role is open only to colleagues currently employed within Kent and Medway NHS Trusts and the Kent and Medway Integrated Care Board (ICB). Nursing vacancies are also open to Student Nurses and Newly Qualified Nurses.
