LPN; Nights

Licensed Practical Nurse (Nights) –
HOMECARE WITH HEART, LLC

Shift: Saturday and Sunday 6 p.m. – 6 a.m. — Pay: $26–$28 per hour (based on experience and availability). Sign‑on bonus: $1,500.

Rapidly growing Home Care agency urgently hiring compassionate and experienced nurses to provide in‑home care for clients. Responsibilities include overseeing quality and expert care of assigned patients.

• Administer medication in accordance with standards of practice.
• Encourage social and rehabilitative skills to promote self‑care and independence.
• Perform client treatments in accordance with the Plan of Care.
• Assist the consumer in learning appropriate self‑care techniques.
• Prepare clinical and progress notes to document care provided.
• Monitor, document, and report any change in client condition to the Clinical Director/Clinical Supervisor.
• Promote activities to achieve a safe and healthy environment.
• Observe aseptic technique when performing client procedures and care.
• Collaborate with other members of the clinical team to develop and improve the Plan of Care.
• Assist the client in achieving established goals in the Plan of Care developed by the RN.
• Maintain productivity according to the agency’s standards.
• Maintain license, CEUs and mandatory employment requirements.
• Participate in client care meetings, in‑services, orientation and staff meetings.
• Provide service in accordance with the agency’s philosophy, standards, policies and scope of practice; participate in Performance Improvement and adhere to HIPAA guidelines.
• Other duties as assigned.

• Valid State LPN License.
• Valid driver’s license; automobile required for travel.
• Annual MUI (Incident) training as dictated by Ohio Medicaid, Ohio Home Care Program and Department of Developmental Disabilities.
• Ability to read and understand agency documents, including Plan of Care, flow charts, client charts, policies and procedures.
• Computer literacy (laptop or tablet).
• Independent judgment, problem‑solving skills and ability to know when to contact a supervisor.
• Excellent customer service orientation and commitment to exceptional client care within agency rules.
