**** What You’ll Do as an RN Case Manager
*** Complete
** home health intake assessments**, medication reconciliation, and care plans
* Collaborate with
** physicians, caregivers, and interdisciplinary teams
*** Educate patients, families, and caregivers on disease management, safety, and recovery
* Maintain
** accurate clinical documentation
** and communication with staff
** Qualifications**
* ** Current RN license in Washington State
*** Minimum
** 2 years nursing experience**;
** home health experience preferred
** but not required
* ** CPR certification
*** Licensed driver with reliable, insured vehicle
* Strong
** communication, clinical judgment, and patient-focused care skills
*** One-on-one patient care in the comfort of their home
