The Community Mental Health Worker 2 functions as part of the Hospitals & Communities Integrated Services (HCIS) team and a specialized Mental Health and Substance Use (MHSU) multidisciplinary team. The role involves ensuring access to timely, evidence-based psychosocial rehabilitation for clients with severe and persistent mental health issues and/or substance use disorders. Responsibilities include supporting clients living independently, performing life skills training, advocacy, supportive counseling, and psychosocial rehabilitation planning.

Reporting to the HCIS Manager, within a psychosocial rehabilitation and harm reduction framework, the Community Mental Health Worker 2 provides basic case management, develops rehabilitation objectives, and engages clients in community settings as active collaborators in their care.

Shift types are days from 08:30 to 16:30. This is a casual position with variable hours (0-37.5 hours/week). Casual employment often leads to permanent or temporary opportunities.
