Your core responsibilities will include performing patient evaluations and contributing to the development of individualized plans of care in collaboration with physical therapists; implementing, monitoring, and progressively adjusting therapeutic exercise programs; guiding gait training, mobility activities, balance tasks, and functional tasks from basic activities to more complex daily living activities. You will administer applicable modalities, assistive devices, and wound care tasks as directed, while ensuring patient safety, infection control, and adherence to regulatory requirements.
Documentation plays a central role—accurate progress notes, outcome measures, and discharge planning communications that support seamless transitions of care. Education for patients and caregivers is a critical component, reinforcing home exercise programs, safety strategies, and strategies for self‑management to sustain gains. You’ll also participate in interdisciplinary rounds, collaborate with the broader care team, and contribute to program development, quality improvement, and patient education initiatives.
