Community Alliance, a leader in community mental health care in Nebraska for over forty years, is seeking a Family Practice APRN to work in our integrated health care setting, providing services to primary care patients. This role is responsible for providing and managing direct patient care in the clinic, including physical examinations, evaluations, assessments, and treatment of acute and chronic illness.
The current patient population is adult, but moving in the direction of serving all ages and providing lifespan care.
The core hours for this position are Monday - Friday, 8:30 AM - 5:00 PM, with one day per week rotation for an evening shift from 5:00 PM - 7:00 PM.
Duties Include- Assumes responsibility for primary care by delivering treatment and management of acute and chronic illnesses, conducting health assessments, physical exams, vaccinations and immunizations, and conducting prevention screenings.
- Act as the champion with Community Alliance for care within the context of an integrated care model, working to grow the practice and improve quality, in collaboration with the members of the Community Alliance team.
- Work collaboratively with all clinic services staff and other Community Alliance program staff in support of direct patient care.
- Current Nebraska licensure as an Advanced Practice Registered Nurse (APRN).
- Preferably have 2 to 5 years of experience in a Family Practice.
- Experience and a demonstrated commitment to care for the needs of a population with mental illness and substance use challenges.
- BLS – Basic Life Support through the American Heart Association upon hire.
