The Skilled Rehab and Home Health team at Good Samaritan St Luke's and St John's seeks a full‑time Physical Therapist to cover Skilled Nursing Facilities and surrounding area Home Health clients. The role involves treating a broad spectrum of conditions, including orthopedic issues, weakness, balance/falls/gait, cervical/back pain, general deconditioning, and other related conditions. The position currently supports one full‑time PT (primary Home Health) and one full‑time PTA (primary SNF).
New graduates are welcomed.
Plan, organize, and deliver individualized physical therapy programs to help clients recover and improve mobility, relieve pain, increase strength, and prevent deformities. Conduct examinations, evaluations, and interventions for patients affected by injury or disease, using appropriate tools and techniques while maintaining safeguards. Document patient information, establish and modify therapeutic plans, and educate patients and families about treatment schedules and home exercises.
Demonstrate current research knowledge in daily care and comply with regulatory and departmental guidelines. Adapt to variable work schedules and participate in departmental, community, and organizational activities. May supervise assistants, aides, and students, providing education and training as needed.
- Education: Appropriate level in accordance with state licensure.
- Licensure: Must be licensed in the state(s) of practice as a Physical Therapist.
- Driver’s License: When applicable, a valid driver's license may be required.
- Competencies: Obtain and maintain required department-specific competencies and certifications.
- Basic Life Support (BLS) for health care providers required or within the first 60 days of employment.
