Overview
An established Home Health/Hospice company is seeking caring RNs in the Houston, Texas area and surrounding counties. We believe in delivering the best care to our patients and families and we are seeking staff who share these beliefs. We are an agency that cares about our staff’s health and their families, and we want to keep them healthy by using increased safety protocols and enhanced patient screenings for infectious diseases.
We partner with leading safety professionals to establish safety protocols for our staff, their families, and our patients.
- Currently licensed with the state of Texas.
- Excellent verbal and written communication skills.
- Experience with electronic medical records is strongly desired.
- Home Health or hospice assessments including consents in the home, hospital, or facility.
- 1 year of Hospice and Home Health experience highly recommended.
- Kinnser experience preferred.
- Be available by phone during normal working hours, when on call, and when on back-up call.
- Performs patient visits according to the Plan of Care or as patient need dictates.
- Delivers care and instruction appropriate to the patient’s plan of care.
- Coordinates care with wound care agencies, doctors and team members via Kinnser or IDG.
- Performs case management activities for assigned patients.
- Responds to verbal and written issues, concerns, or complaints from home care patients, families, physicians, and nurses.
- Comprehensive assessments of patients utilizing the OASIS data set or Hospice HIS data set.
- May entail GIP care.
- Meet with the IDT/case conference team once every other week.
- QA for clinicians on the care team.
- Experience working with children is highly considered.
- Coordinate schedule to maximize time during the day.
- May require driving outside of the residential area.

Education- Bachelor's (Preferred)
- Nursing: 1 year (Preferred)
- Home health and/or hospice: 1 year (Preferred)
- BLS Certification (Preferred)
- RN Texas or Compact state license
