Overview
Care Med Health Solutions is a facility-based team of medical providers delivering services to residents of long-term care facilities. We strive to provide quality care in partnership with each facility to achieve the best possible outcomes for the elderly population. An interdisciplinary care team supports medical care in these settings, and this role offers opportunity to positively impact residents' life and wellbeing.
We are currently seeking Wound Care Nurse Practitioners to provide wound services to patients living in long-term care facilities.
Responsibilities- Provide wound care services for long-term care patients residing in skilled nursing facilities.
- Prescribe appropriate medications, including controlled substances in accordance with Kansas law.
- Order, review, and/or follow up with treatments and diagnostic testing.
- Communicate with the nursing home staff regularly and as needed to discuss changes in condition and care plans.
- Collaborate with the primary care physician.
- Current, unrestricted NP license in the state of Kansas.
- Board Certification.
- DEA licensure (Optional).
- Clinical background in adult, family, or geriatric specialties.
- Clinical background in wound care, including chronic, debilitating, stalled, or infected wounds.
- Comfortable diagnosing and treating diabetic, arterial, venous, and mixed etiology wounds or Kennedy Ulcers.
- Comfortable staging pressure injuries according to the NPIAP.
- Familiar with wound care best practices and advanced wound care products (collagens, alginates, foams, silvers, etc.).
- Experience with wound VACs and other DME such as pressure-relieving/reducing devices.
- Strong preference for wound certification or openness to obtaining certification.
- Experience working in a long-term care setting.
- Ability to work with an EMR.
- Ability to work within an interdisciplinary care team to coordinate patient care.
